Immunizations
Just like good nutrition, exercise and regular check-ups, vaccines play a vital role in keeping you healthy throughout your life – from childhood through your senior years.
KCHD provides vaccinations for both children and adults, plus immunization records.
*If you are moving to Tennessee and need to transfer for your vaccine records, please call (865-215-5150). To request your vaccination records, visit https://knoxcounty.jotform.com/team/ip/ImmRequest
